Day of Your Appointment

pre-screening questions

Before your appointment, we will need to ask you a few pre-screening questions.  If you answer yes to any of the following questions, we must cancel your appointment and reschedule you for two weeks from now.

• Have you or anyone you have been in contact with had a fever over 100.4?
• Have you or anyone you have been in contact with had a cough, shortness of breath, sore throat, new loss of taste or smell, chills, Nausea, vomiting, or diarrhea?
• Have you or anyone you have been in contact with tested positive for COVID-19, or been told by a health care provider to seek testing for COVID-19?
